Vaccine Information Sheet – FLU SHOT

The CDC has released the final version of the Vaccine Information Sheet (VIS) for the Inactivated H1N1 Flu Vaccine (attached). The final version and the preliminary version are very similar.  The new version includes some grammatical and wording changes. In addition, some items have been moved from one section to another. The most significant change is that a paragraph was added to Section 3 at the bottom.

The packets you have received/will receive do not contain this final version (they contain the preliminary form). Feel free to replace the preliminary version with this new version prior to sending to parents. If you have already sent the packets or do not have the resources to make the copies, you may also consider posting this new version to your website for parents to view.

SPANISH version - HCPH is currently translating the VIS for the Flu Shot and will email it out as soon as it is available (likely Monday 10/5).

Vaccine Information Sheet – NASAL SPRAY

CDC also released the VIS for nasal spray (attached). Again, if time and resources allow, this could be copied and included in the parent packet or posted on your website.
